Tibial slope after Medial Opening-Wedge High Tibial Osteotomy: Is plate positioning the biggest issue?
نویسندگان
چکیده
Objetive: One major cause of Medial Opening-Wedge High Tibial Osteotomy (MOW-HTO) failure is inadvertent Posterior Tibial Slope (PTS) increase. To avoid this complication, posterior placement of the wedge plate is traditionally recommended. However, in the attempt of achieving such positioning, surgeon may encounter some undesired consequences, like medial collateral ligament disruption, instability or neurovascular injury. The purpose of this study is to evaluate the correlation between plate position in the sagittal plane and posterior tibial slope, in patients undergoing MOW-HTO. Methods: Forty-three patients undergoing MOW-HTO were radiographically evaluated, before and after surgery, to determine posterior tibial slope changes and plate position. to 10.3 ± 4.9 degrees (p = 0.0005)-this was not influenced by position or size of the plate. High values of posterior slope before surgery were correlated with greater increases after surgery. Conclusion: Plate positioning and wedge size were not related to PTS change, but patients with high preoperative PTS values showed a significantly greater increase in postoperative PTS. Level of evidence: Level 4 – case series.

Opening wedge tibial osteotomy: the 3-triangle method to correct axial alignment and tibial slope.
BACKGROUND Although a change in tibial slope may occur during a medial opening wedge osteotomy, calculations have not been defined to address this problem. The authors investigated geometric factors important to correct axial alignment and tibial slope during osteotomy. PURPOSE To calculate, through 3-dimensional analysis of the proximal tibia, how the angle of the opening wedge along the ant...
متن کاملHigh tibial osteotomy: closed wedge versus combined wedge osteotomy
BACKGROUND High tibial osteotomy is a common procedure to treat symptomatic osteoarthritis of the medial compartment of the knee with varus alignment. This is achieved by overcorrecting the varus alignment to 2-6° of valgus. Various high tibial osteotomy techniques are currently used to this end. Common procedures are medial opening wedge and lateral closing wedge tibial osteotomies. The latera...

OBJECTIVE To measure the variation in posterior tibial slope angle and patellar height in patients who underwent proximal tibial valgus-producing osteotomy using the medial-opening wedge technique. METHODS Anteroposterior panoramic radiographs of the lower limbs and lateral radiographs of the knee obtained before and after tibial valgus-producing osteotomy on 46 patients with unicompartmental...
متن کاملConventional plate and screws in medial opening-wedge high tibial osteotomy: are they sufficiently stable? A retrospective study☆
OBJECTIVE Opening-wedge osteotomy of the proximal tibia is a widely performed procedure for treating medial gonarthrosis in active patients and in the presence of varus malalignment of the lower limb. The fixation method is controversial, and the use of conventional implants has been abandoned in favor of implants with more modern locking screws. The aim of the present clinical study was to ass...
متن کاملModified retro-tubercle opening-wedge versus conventional high tibial osteotomy.
Despite the fact that common surgical techniques for the treatment of genu varum usually correct the malalignment in the affected knee, these methods have significant complications and cause problems in the long term. Retro-tubercle opening-wedge high tibial osteotomy is among the newer techniques for the treatment of genu varum. The goal of this study was to compare the results of retro-tuberc...
